I, ___________________________, author (or co-author and agent for all co-authors), give permission to Wisdom of Practice: An Online Journal of Action Research (WOPOJAR) to publish the paper entitled:

I attest that this paper is my (or our) original work, that no form of the paper has been previously published, and that copyright has not previously been assigned to another publishing or distribution agent.  I (we) will not submit this paper to any other publisher for review or publication.

I attest that I  (we)  have approval from the Human Subjects Committee  (HSC)  of my university's Institutional Review Board  (IRB)  to publish this  research as submitted.  For example, pseudonyms have been used in place of real names, if that was specified in the HSC application, and participants and confidentiality have been protected as agreed.

I  (we)  grant publication rights exclusively to WOPOJAR.  An exception is permitted in case I (we) might be invited to contribute all or portions of the paper to a collection or derivative work by another publisher or organization (e.g., as a chapter in a case book).  If this should occur, it will be my responsibility to ensure that the reproduction includes the following attribution:
