INTERNSHIP PROGRAM

Student Internship Registration Forms

The following four forms must be completed, signed, and turned in to Danette Knowlton (VCLS 308) before the tenth day of the semester to ensure the student is properly registered for the Internship:

  1. Internship Learning Agreement  This agreement describes the Internship site, work responsibilities, and the academic requirements that must be completed by the student Intern to earn academic credit. The student Intern, employer, and faculty sponsor sign the agreement. This form must be completed before the Faculty Sponsor can sign a Supplemental Registration Form.

  2. Participation Agreement   This agreement must be signed by the student Intern, employer and faculty sponsor before registering for the academic credit.

  3. Release of Liability   This form must be signed by the student Intern before registering for the academic credit.

  4. Supplemental Registration Form   This form is required to register for Internships arranged on an individual basis through a WSU Vancouver faculty member.

Internship Evaluation Forms

  1. Student Evaluation of Internship Experience   Form is completed and signed after the conclusion of the Internship. Student Intern must return this form to the faculty sponsor via mail or fax by finals week of the semester in order for a pass/fail grade to be assigned.

  2. Final Employer Evaluation   Student Intern is responsible for providing a blank evaluation form to the employer. This form must be completed and signed after the conclusion of the Internship. Student Intern must return this form to the faculty sponsor via mail or fax by finals week of the semester in order for a pass/fail grade to be assigned.
