Medical Benefits

You must live in the medical plan's service area to join the plan. Plans available in the Vancouver/Portland area include the Uniform Medical Plan, Kaiser Permanente Classic, Kaiser Permanente Value, and AETNA. Uniform Medical Plan

UMP PPO is a preferred provider organization. That means you can choose your own providers. You don’t have to choose a primary care provider who acts as a “gatekeeper” to manage your care, and you don’t need to get a referral from UMP to see a specialist. You will receive the highest level of benefit and pay the lowest amount out of your pocket if you see network providers.

With choice comes responsibility! To get the most out of your benefits, you need to know what services UMP PPO covers and whether your provider is in our network.

Kaiser Permanente Classic

The Classic plan offers Kaiser's broadest coverage and lowest copayments for office visits and other services. What's more, there's no annual deductible. Office visits, have just a $10 copay. Generic and brand-name prescriptions have a $10 and $25 copay, respectively.

Kaiser Permanente Value

If you're concerned about monthly costs, the Value plan has the lowest premium you can get. Rates are just $59 per month for individuals, $103 for individual and child, $128 for individual and spouse, and $172 for individual and family. Copayments are slightly higher than the Classic plan for office visits. However, you'll receive the same valuable services.

AETNA

AETNA is structured like a PPO in that you may select any provider wihtin the network, and structured like a managed care plan with regards to cost. There is no annual deductible with a copay of $10 for an office visit. In addition, this is a self referral plan; therefore, there is no need for a referral from a PCP for specialty care. Prescription drug benefits are $10 copay for generic, $25 for preferred brand, and $40 for nonpreferred brands.
